2011 Membership Tip Off Entry Forms
You must use attached/website forms and return by March 7. Late entries are accepted until March 11 with $50 late fee if openings exist. Please contact Sport Director Andy Fishman if you plan to submit a late entry at 513-477-0991 a.fishman@lakotaonline.com or Ohio Governor, Mark Hecquet, mhecquet@butlercountyvb.com.
Additionally, registration must be emailed /mailed with an entry fee of $200 payable to Ohio AAU or submitt payment online below. Checks, rosters and registrations can be mailed to the address below.
