If you are unable to complete the Roster portion of the Entry Form at this time but would still like to enter the tournament, please complete part 1 of the entry form and submit your payment. This will reserve your team's entry in the tournament.
You will not be entered into tournament without a completed Entry Form and Money Order or Cashier's check. Please make the Money Order or Cashier's check payable to Ohio AAU.
